Thank you for your dedication to the Transitional Housing Project of the Elder Abuse Institute of Maine. As part of our requirements to our federal funders, we are asked to collect data on the activities and outcomes of all of our partner organizations, which is ultimately reported to Congress.
The above survey will help us collect this data and ensure accurate reporting to the U.S. Department of Justice.
We ask that all partner agencies complete this survey quarterly to be submitted by the following dates: July 1, October 1, January 1, April 1. This data will be aggregated and submitted to the Department of Justice for the Performance Report (due twice yearly) and the Recovery Act reporting (due quarterly). The first report you will submit will be on July 1, 2010 and should contain data from the beginning of your work.
Please fill in the sections that pertain to activities in which you are engaged only. This report is NOT cumulative: that is, each report you fill out should contain information for that reporting period only. Please make sure you fill in the box that indicates the reporting period for the current report you are submitting.
